Alll participants in OCD Volleyball Events (league or tournament) will be required to sign a Release of Liability Agreement prior to playing. There is no cost for this as OCD Volleyball pays directly for insurance rather than passing the costs on to its players. The waiver only needs to be filled out ONCE per season and covers you for all OCD Events for the season. 

 

By signing this document electronically, you agree your electronic signature is the legal equivalent of your manual signature on this Agreement and you consent to be legally bound by this Agreement's terms and conditions.

IMPORTANT INFORMATION FOR PLAYERS UNDER 18: Participants under the age of 18 will be required to have a parent or legal guardian also sign the Release of Liability Agreement. Full instructions HERE or read below. 

STEP 1: The Player must fill out the waiver

Parents cannot fill out the waiver on behalf of the player. The actual player must execute and sign the waiver.

  • Go to http://bit.ly/ocdwaiver
  • Enter your email address
  • Check your email and click on the link
  • Fill out the waiver online (do not download or print it)
  • Submit the waiver

Because you are under 18, parents must also sign the waiver.

STEP 2: Parent signs the waiver

  • Check the email you used to complete the waiver for a confirmation email. This confirmation email will include a PDF attachment of the filled out waiver
  • Print the attachment
  • A parent or legal guardian, not a friend or the minor, needs to fill out and sign the bottom portion of the waiver. Bring the printed waiver with the parent signature to the event and hand to the tournament or league director prior to playing

** Players under 18 will not be allowed to play without this. No paper form will be available at the tournament/league site. Verbal permission or permission other than this legally binding waiver will not be accepted. No exceptions will be made.
